Medical Referrals for Professionals

Eyecare Medical Services are part of the NHS England increasing capacity framework. We are a free NHS service catering for all adults with macula disease. We welcome referrals for adults from most regional areas with suspect age related macular degeneration (AMD), retinal vein occlusion, diabetic retinopathy.

We welcome referrals for adults over 18yrs from most regional areas with suspect macula degeneration, vein occlusion, diabetic retinopathy. We accept switch patients. Some exceptions do apply.

The referral form must be completed by a healthcare professional.

Once completed; we can accept the form by either fax or secure email. You must use an NHS net account if you choose to email the form to us.
